Midlines are used to administer medications that are non-irritating to the vein and provide greater flexibility than a PIV. They can be used for either short or longer intervals on patients with difficult vein access (DIVA) and can reduce the number of patient sticks as a substitute for multiple PIVs that do not last the course of treatment. Midline catheter placement typically begins in a vein located just above the elbow but does not extend into a central vein or near the heart. When are Midlines Used?
Midlines are used as good options for short or longer intervals on patients with difficult vein access, including scenarios where PIVs fail frequently. Prior to placing a Midline, a comprehensive review of the patient’s medical record along with a detailed physical assessment should be performed to gain a thorough understanding of their medical history and care plan. This ensures there are no contraindications and that the appropriate vascular access device is placed. How do you care for a Midline?
Sterile techniques must always be used. Dressings over the Midline insertion site should be inspected daily and changed regularly, typically once per week. If the dressing becomes dislodged, wet, or soiled, a healthcare provider should be notified immediately. The site should always be kept dry when you bathe or shower, and you can use a PICC line shower covering or securely tape plastic wrap when bathing. Patients should limit or refrain from activities that may disturb or accidentally dislodge the Midline device, including avoiding repetitive movements and not lifting more than 10 pounds with the arm in which the Midline is placed.
Can Midlines be placed in Home Settings?
Yes, Midlines can be safely placed in patients’ homes, which allows treatment to occur in a familiar and comfortable environment provided sterile environments during the procedures can be maintained. For example, pets must not be present during the procedure. Placing a vascular access device at the patient’s bedside – no matter where that is – can improve patient outcomes, increase patient satisfaction, and reduce costs such as transportation, and help avoid the possibility of exposure to other patients, viruses, and bacteria.
